PvE: Pulling without mantras/buffs already up (MetaBot)
PvE: Running Undefeated Mantra alongside a Cleric's Light of Protection (they don't stack) (MetaBot)
PvE: Casting Dark Crush outside its 2s follow-up window (MetaBot)
PvP: Using Barrier Spell expecting it to stack with other invulnerability or stop executes (MetaBot)
